Ayushman Bharat Strengthens India’s Healthcare Backbone

Government says flagship health scheme is reducing treatment burden on poor and middle-class families while expanding digital and rural healthcare access

Desk: India’s healthcare landscape is witnessing a major transformation through the Ayushman Bharat initiative, with the Centre aggressively expanding affordable medical coverage, digital health infrastructure and primary healthcare access across the country.

According to a detailed background note released by the Press Information Bureau (PIB), the Ayushman Bharat-Pradhan Mantri Jan Arogya Yojana (AB-PMJAY) has now become the world’s largest public healthcare scheme, aimed at ensuring quality treatment for economically vulnerable families without financial hardship.

Launched on September 23, 2018, under the broader vision of universal healthcare, the scheme provides annual health insurance coverage of up to ₹5 lakh per eligible family for secondary and tertiary hospitalization. The programme was designed to reduce out-of-pocket medical expenditure, which has historically pushed millions of Indian households into debt every year.

Government data shows that more than 42 crore Ayushman cards have already been issued to beneficiaries under AB-PMJAY as of October 2025, while over 12 crore vulnerable families are currently covered under the scheme. More than 86 lakh senior citizens have also been enrolled after the government expanded the programme for citizens aged above 70 years.

Officials say the scheme is gradually bridging the healthcare gap between urban and rural India by ensuring cashless treatment access through an expanding network of empanelled hospitals across states.

A Shift Towards Universal Healthcare

The Ayushman Bharat mission was conceptualized following the National Health Policy 2017, which identified the growing healthcare challenges arising from changing disease patterns, increasing lifestyle diseases and unequal medical access across regions.

Unlike traditional health insurance programmes, Ayushman Bharat was structured as a multi-layered healthcare ecosystem combining insurance coverage, primary healthcare centres, digital health services and medical infrastructure expansion.

The scheme operates through four major pillars:

  • Ayushman Bharat-Pradhan Mantri Jan Arogya Yojana (AB-PMJAY)
  • Ayushman Arogya Mandirs (AAM)
  • Ayushman Bharat Digital Mission (ABDM)
  • PM-Ayushman Bharat Health Infrastructure Mission (PM-ABHIM)

Together, these initiatives are aimed at strengthening healthcare delivery at primary, secondary and tertiary levels.

Financial Relief for Crores of Families

One of the most significant impacts of AB-PMJAY has been the reduction in catastrophic medical spending among low-income households.

Government estimates indicate that the scheme has helped families save more than ₹1.52 lakh crore in medical expenses. Under the programme, beneficiaries can avail cashless treatment for serious illnesses including cardiac surgeries, cancer treatment, kidney disorders, orthopaedic procedures and emergency care.

Health policy experts believe that the programme has created a much-needed financial safety net, particularly for rural families who earlier depended heavily on private borrowing or asset sales during medical emergencies.

Recent government data also revealed that private hospitals under AB-PMJAY have authorized hospital admissions worth over ₹1.15 lakh crore till January 2026.

Expansion of Rural and Primary Healthcare

Apart from insurance coverage, the government has simultaneously focused on strengthening primary healthcare infrastructure through Ayushman Arogya Mandirs.

These centres, earlier known as Health and Wellness Centres, have been developed by upgrading existing Sub-Health Centres and Primary Health Centres across India. Their objective is to provide preventive, promotive, curative and rehabilitative healthcare services closer to local communities.

The centres are now offering services related to diabetes, hypertension, oral cancer screening, maternal healthcare, mental health support, palliative care and teleconsultation facilities.

As per government records, over 1.62 lakh Ayushman Arogya Mandirs have already become operational nationwide.

Healthcare experts say this decentralization of primary healthcare could play a critical role in reducing pressure on district hospitals and tertiary healthcare institutions over the long term.

Digital Healthcare Ecosystem Expanding Rapidly

India is also witnessing rapid healthcare digitization under the Ayushman Bharat Digital Mission (ABDM).

The initiative aims to create an integrated digital health infrastructure connecting hospitals, clinics, laboratories and patients through a unified digital ecosystem. Citizens receive ABHA (Ayushman Bharat Health Account) IDs, enabling secure digital storage and sharing of medical records.

According to official data, nearly 80 crore ABHA IDs have already been created, while crores of electronic health records have been digitally linked through the platform.

Officials believe digital integration will significantly improve efficiency, reduce paperwork and help patients access treatment seamlessly across healthcare facilities.

Post-Pandemic Healthcare Infrastructure Push

The COVID-19 pandemic exposed major gaps in India’s healthcare infrastructure, especially in critical care capacity, laboratories and disease surveillance systems. Following the pandemic, the government launched the PM-Ayushman Bharat Health Infrastructure Mission (PM-ABHIM) in 2021.

The mission focuses on strengthening healthcare infrastructure from village-level health centres to district hospitals and advanced laboratories.

The programme includes investments in disease surveillance networks, intensive care facilities, public health laboratories and emergency preparedness systems to improve India’s response capability during future health crises.

Towards “Viksit Bharat 2047”

Government officials say Ayushman Bharat is not merely an insurance scheme but a long-term structural reform aimed at building an inclusive healthcare ecosystem for a developed India.

As healthcare inflation rises and lifestyle diseases increase rapidly across both urban and rural India, the expansion of affordable healthcare coverage is expected to remain a key policy priority in the coming years.

With crores of beneficiaries already enrolled, expanding hospital networks, digital integration and strengthening grassroots healthcare services, Ayushman Bharat is increasingly being viewed as one of independent India’s largest healthcare transformation programmes.
